INSTRUCTIONS

If applicant was nationalized in a different temple, they must show proof of nationality by including a copy of their nationality paperwork or a temple identification card.

If not already nationalized, we will need a copy of your state ID, and/or birth certificate and if you legally changed your name, your name change documentation. (You can upload and your documents below).
Two (2) Passport Size Photos (Men wearing maroon Fez with tassel over left shoulder and white shirt. Women wearing white, red or green turban). (You can upload and your documents below).
$95.00 Minimum Donation for Adults Cards, $75 Minimum Donation for minors. If paying by Postal Money Order, make it out to Moorish Science Temple of America PO Box 455, Bronson Florida, [32621]. We also accept PayPal and Zelle.
To mail with Tracking# add $5.00. For any other special mailing i.e. Overnight, Special Delivery, find out the cost and add additional fiat accordingly.
