1). If applicant does not have a Visa/Passport they must provide a copy of their state ID or birth certificate.
2). If applicant does not have a Visa/Passport but they have been nationalized, they must show proof of Nationality by including a copy of their nationality paperwork from a temple or a temple identification card.
3). At least 3 samples of your signature double spaced and preferably in red ink (no black ink) on plain white unlined paper as bold and thick as possible. Thumb print is permissible.
4). Two (2) Passport Size Photos (Men wearing maroon Fez with tassel over left shoulder and white shirt. Women wearing white, red or green turban)
5). $95.00 Minimum Donation for Adults Cards, $25 Minimum Donation for minors. Postal Money Order preferably made out to Moorish Science Temple of America. We also accept PayPal and Zelle.
6). To mail passport card with Tracking# add $5.00 to total cost. For any other special mailing i.e. Overnight, Special Delivery, find out the cost and add additional fiat accordingly.
